Once you submit the application below, you should receive a confirmation email to let you know we have received your registration form.  In about 5-10 business days you will recieve temporary donation envelopes so you may begin offering to St. Monica's.  In about 4-6 week you will receive your personalized donation envelopes with your official Church Envelope Number.
